Population health provides an opportunity for health care systems, agencies, and organizations to work together in order to improve the health outcomes of the communities they serve. Some definitions of population health emphasize outcomes, some focus on measurement, and others emphasize accountability. Chronic kidney disease in the United States places a high burden upon society and health systems. Given its uneven distribution across populations and that the disease is largely preventable, chronic kidney disease is an important focus for population health. Drs Mary Reich Cooper, Tara McKinley, and Joseph Cirrincione will discuss population health with a focus on chronic kidney disease.

Most improvements in health care outcomes are attributed to clinical, operational, or financial process changes. However, patients can also play a considerable role in improving their care. Listen in as we explore patient empowerment and shared decision-making concepts, specific to patients suffering from kidney disease.
