Population health provides an opportunity for health care systems, agencies, and organizations to work together in order to improve the health outcomes of the communities they serve. Some definitions of population health emphasize outcomes, some focus on measurement, and others emphasize accountability. Chronic kidney disease in the United States places a high burden upon society and health systems. Given its uneven distribution across populations and that the disease is largely preventable, chronic kidney disease is an important focus for population health. Drs Mary Reich Cooper, Tara McKinley, and Joseph Cirrincione will discuss population health with a focus on chronic kidney disease.

Patient empowerment is an important concept in the care of patients with chronic kidney disease. This presentation discusses the foundational aspects of motivational interviewing, basic counseling techniques, and the importance of this collaborative counseling method in this population. By communicating and empowering patients, many outcomes have been shown to improve in chronic kidney disease.
With the advancement of technology throughout the 21st century, we have seen a growing role for the utilization of information technology applied to health and health care. Health information technology (HIT),including HIT used by patients, can play an important role in patient empowerment by creating new opportunities for patients and families to participate actively in their care management. In the Nephrology setting, HIT can help patients with chronic kidney disease be informed, engaged, committed, and collaborative in managing their health and care.
