In this webinar, Dr. Calvin Meaney and Dr. Katie Cardone discuss the role of inpatient and outpatient clinical pharmacy services and the medication therapy challenges in managing patients with chronic kidney disease (CKD). Given recent practice developments (e.g. polypharmacy reviews and pharmacist prescribing), they provide a current synthesis of evidence-based characteristics and outcomes of clinical pharmacy practice in treating patients with CKD.
