Empowering Patients Through Health Literacy

As part of our Patient Empowerment series, this presentation brings you concepts and resources to help improve health literacy among your patients, especially those with chronic kidney disease, as they manage the complexities of their care regimen. Health literacy is crucial for dialysis and transplant patients so they can be empowered to participate in shared decision-making. Effective communication strategies lead to better patient engagement, and engaged patients can better deal with the many challenges they face in living with kidney disease.
