The Role Of The Pharmacist In Chronic Kidney Disease
Clinical pharmacy services have the potential to contribute significantly to the multidisciplinary team by providing safe, effective, and economic care for patients. Given recent practice developments (e.g. polypharmacy reviews and pharmacist prescribing), there is a need to provide a current synthesis of evidence-based characteristics and outcomes of clinical pharmacy practice in treating patients with chronic kidney disease (CKD). Dr. Calvin Meaney and Dr. Katie Cardone outline the role of both inpatient and outpatient clinical pharmacy services as well as discuss medication therapy challenges in managing patients with CKD.

Albany College of Pharmacy and Health Sciences*
Katie Cardone is an associate professor and pharmacy program director in the Department of Pharmacy Practice at Albany College of Pharmacy and Health Sciences. She received her doctor of pharmacy degree from Albany College of Pharmacy and subsequently completed a fellowship in nephrology pharmacy at Albany Medical Center and Albany College of Pharmacy. Dr. Cardone has had clinical practice sites in dialysis and outpatient nephrology for over a decade and serves as the director of clinical pharmacy services at The Collaboratory, a public health outreach office housing community health workers and a public health pharmacy team. She serves as residency program director for the College’s PGY2 ambulatory care residency. Her research program focuses on optimizing medication outcomes in patients with kidney disease. Dr. Cardone a board certified ambulatory care pharmacist and is a fellow of the National Kidney Foundation, the American Society of Nephrology and the American College of Clinical Pharmacy.
Calvin J. Meaney, PharmD, BCPS

Dr. Meaney is a Clinical Associate Professor in the Department of Pharmacy Practice at the University at Buffalo School of Pharmacy and Pharmaceutical Sciences. He received his Doctor of Pharmacy from the University at Buffalo, followed by residency training at the University of Maryland Medical Center and fellowship in clinical pharmacology back at the University at Buffalo. He is clinical pharmacist-scientist in the field of nephrology. His clinical practice is at the Erie County Medical Center, the Regional Center of Excellence in Transplantation in Kidney Care, where he precepts pharmacy students and residents. Dr. Meaney‘s research is focused on the optimization of drug therapy through the application of quantitative clinical pharmacology. He leads multidisciplinary research groups focused on anemia in chronic kidney disease and drug-induced nephrotoxicity.
Lisa Voigt, PharmD, BCPS, BCCCP

Lisa Voigt, PharmD, BCPS, BCCCP is a PharmD who works as a Senior Medical Science Liaison in Nephrology with Otsuka Pharmaceutical Development & Commercialization, Inc. Prior to joining Otsuka in 2019, Dr Voigt practiced as a Medical Intensive Care Unit and Surgical Intensive Care Unit Pharmacist for 15 years at a tertiary medical center in Buffalo, NY. She earned her Bachelor of Science degree in Pharmaceutics and PharmD from the SUNY Buffalo School of Pharmacy and Pharmaceutical Sciences. She completed a Post-Graduate year 1 Pharmacy Residency and Post-Graduate year 2 Critical Care/Emergency Medicine Pharmacy Residency at the University of Rochester Medical Center in Rochester, NY. Dr Voigt is a Past-President of the New York State Council of Health-system Pharmacists and has served on the Board of Directors for the past 10 years. She is also Past-President of the Western New York Society of Health-system Pharmacists. She is an Adjunct Assistant Professor at SUNY Buffalo, School of Pharmacy and Pharmaceutical Sciences. Dr Voigt holds board certifications in pharmacotherapy as well as critical care pharmacotherapy.
