Discover The PKD Foundation’s ADPKD Centers Of Excellence Program
Background
The PKD Foundation’s Centers of Excellence (COE) program is built upon the belief that the best way to provide ADPKD-centered care is through patient-focused, comprehensive care with the coordination and support of an integrated care team. The program’s mission stands on three pillars:- understanding Polycystic Kidney Disease (PKD) through research
- ensuring better care for all individuals with Autosomal Dominant Polycystic Kidney Disease (ADPKD)
- educating and empowering the community.
Why Attend This Webinar?
- Take a deep dive into the PKD Foundation COE initiative to coordinate and standardize specialty care for patients with PKD, which includes the professional network created between Centers of Excellence and designated Partner Clinics across the United States.
- Be part of the PKD professional community that is empowering patients, advancing research, and optimizing care for those affected by PKD in hopes of improving outcomes.
- Receive information on how to apply to the program and hear from the PKD Foundation about the critical deadlines for the 2023 application cycle.
