Understanding the ADPKD Patient Journey: From Primary to Specialty Care

Background 

Patients with ADPKD often experience both renal and extra renal manifestations of the disease.1 Although nephrologists play a central role in the management of ADPKD, patients often rely on their primary care physicians for initial and intermittent care. Additionally, other specialties like urology are often involved if certain complications are present.

Maryam Gondal

Maryam Gondal, MD, MBBS

Assistant Professor of Medicine at Yale School of Medicine
Dr. Maryam Gondal is a graduate of Aga Khan University in Karachi, Pakistan. She did her Internal Medicine Residency at Indiana University and subsequently completed

fellowship training at Yale University. She is involved in PKD center of Excellence at Yale and takes care of a substantial number of patients with ADPKD. Currently, she is a Sub-Investigator for one of the Autosomal Dominant Polycystic kidney clinical trials at Yale.

Dylan Poche

Dr. Dylan Poche earned his medical degree from Louisiana Health and Sciences Center in New Orleans and then completed three years of residency training in

Medicine. He is board-certified by the American Board of family medicine and a member of the American Academy of Family Physicians.

Elan Salzhauer

Elan Salzhauer, MD

Medical Director, Urology at St. Joseph’s Hospital and Medical Director, Intrepid Lane Surgery Center in Syracuse, NY
Dr. Elan Salzhauer is a Board Certified Urologist specializing in robotic and laparoscopic surgery. He treats a wide range of urologic conditions, with a subspecialty

on Urologic Kidney disease including Kidney Cancer, Stone disease, Renal and Ureteral reconstruction and Autosomal Dominant Polycystic Kidney Disease. In addition he specializes in Prostate Cancer and BPH surgery. Dr. Salzhauer believes in a multi-disciplinary approach to the treatment of these disorders and regularly brings in Nephrology, Medical Oncology, and Radiation Oncology specialists to find the optimum outcome for his patients.

Dr. Salzhauer currently serves as the Medical Director of the Urology Department at St. Joseph’s Hospital and the Medical Director of the Intrepid Lane Surgery Center both in Syracuse NY.

Dr. Salzhauer received his undergraduate degree from Yale University, his Medical School training from SUNY Upstate Medial School and his Urology training from SUNY Downstate Medical School in New York City including training at Memorial Sloan Kettering Cancer Center. Dr. Salzhauer’s research has been published in the Journal of Urology, the British Journal of Urology and the Journal of Pediatrics. He has presented his work at several international meetings and he is a member of several major medical and urological associations. He has won several awards including a prestigious teaching award and continues to educate other physicians as the Urology instructor for the St. Joseph’s Hospital Family Practice residency program.

Dr. Salzhauer and his wife Anne have four children and enjoy living in central New York. He has been in practice since 2006.
